Insurance

Health Lab Physical Therapy + Fitness is currently accepting the following insurance plans:

  • Aetna
  • Premera BCBS Alaska
  • United Healthcare
  • EBMS
  • TRICARE Select
  • TRICARE Prime (with referral + auth)
  • Workers' Compensation
  • Medicare


If your insurance provider is not on our list, please call to see if we are in the process of credentialing, or if we are able to apply to become in-network with them. We can provide you with a Superbill if you have out-of-network benefits.

Cash/Self Pay

If you plan to pay cash for your services at Health Lab Physical Therapy and Fitness, we would love to work with you! All visits are one hour, and one on one with your PT.


Payment Options

  • Single session: $175
  • Packages: 
    • $165/session for 4 sessions
    • $155/session for 8 sessions
  • Wayfinder Fitness Members
    • $150/session


You can use your HSA, HRA, and FSA to pay for physical therapy services!
